How to Enroll in/Increase Life Insurance

You may enroll in Optional Life coverage at one-half or one times your annual salary without providing evidence of good health if you have experienced a Change in Status.  You may do so by completing a Benefit Change form (HR 105) and submitting it to your Human Resources office within 60 days of the Change in Status.  You may also increase your Optional Life coverage by one salary increment (up to 5 times your salary) if you have experienced a Change in Status.  You may do so by completing a Benefit Change form (HR 105) and submitting it to your Human Resources office within 60 days of the Change in Status.  The Benefit Change form is located at http://sago.tamu.edu/shro/Forms/105.pdf

You may apply to enroll in or increase your Optional Life at any time during the year by completing an Evidence of Insurability Application and mailing the form to the life insurance carrier for processing.  Send the form directly to the carrier at the address listed on the form.  The form is located at: http://sago.tamu.edu/shro/Forms/Dearborn E0fI.pdf   In addition, complete a Life Insurance Enrollment form, HR 138, located at http://sago.tamu.edu/shro/Forms/138.pdf .  Send this form to your Human Resources office.  You will be notified by the Life Insurance Carrier as to the status of your application.   

To enroll your dependents in Dependent Life Insurance within 60 days of a Change in Status, complete a Dependent Enrollment Form/Certification, located at http://sago.tamu.edu/shro/Forms/101.pdf .  Send this form to your Human Resources office within 60 days of the Change in Status.

To apply for Dependent Life coverage without a Change in Status, complete an Evidence of Insurability form, located at http://sago.tamu.edu/shro/Forms/DearbornEofI.pdf

Send this form directly to the Life Insurance Carrier, at the address listed on the form.  In addition, complete a Dependent Enrollment/Certification form, HR 101, located at http://sago.tamu.edu/shro/Forms/101.pdf .  Send this form to your Human Resources office.  You will be notified by the Life Insurance Carrier as to the status of your application. 

You may cancel or decrease your Optional Life coverage at any time by submitting a Benefit Change form, HR 105(http://sago.tamu.edu/shro/Forms/105.pdf ).  You may cancel your Dependent Life coverage at any time by submitting a Dependent Enrollment Form/Certification, HR 101 (http://sago.tamu.edu/shro/Forms/101.pdf ).
